Who it's for
Anyone told their ultrasound shows a fatty liver — now called MASLD, or MASH when the liver is inflamed — especially alongside diabetes, prediabetes or extra weight, which is how it usually comes.
If you've been told you have cirrhosis, or your liver tests are well out of range, see your doctor before you change your diet. Advanced liver disease needs different nutrition, including enough protein.
What the evidence shows
The 2024 guideline from the European liver, diabetes and obesity societies sets the targets: a sustained weight loss of at least 5% to reduce liver fat, 7–10% to improve inflammation, and 10% or more to improve fibrosis. For someone weighing 80 kg, 7% is about 5½ kg.
The same guideline recommends a Mediterranean-style diet, cutting down on ultra-processed food rich in sugar and saturated fat, and avoiding sugar-sweetened drinks — together with more than 150 minutes a week of moderate exercise such as brisk walking.
In a year-long study of 293 people with biopsy-proven steatohepatitis, 90% of those who lost 10% or more of their weight saw the inflammation resolve, and 45% saw the scarring improve.
Easy swaps
- Cold drink → water with lemon and mint, or soda water.
- Sweet lassi → plain lassi.
- Pakoras with evening chai → popcorn, roasted chana or a piece of fruit.
- Paratha and fried egg → roti and an omelette made with a little oil.
Common questions
- Can a fatty liver go back to normal?
- Often, yes. Liver fat falls with about 5% weight loss, inflammation improves at 7–10%, and even scarring can improve at 10% or more. The earlier the stage, the better the chance.
- Should I try keto for fatty liver?
- Low-carbohydrate and low-fat diets reduce liver fat about equally when calories come down, and the European guideline found too little evidence to recommend very-low-carbohydrate keto diets for fatty liver. The Mediterranean pattern has the strongest support and is easier to keep up.
- Is there a medicine for fatty liver?
- Some medicines help — semaglutide and tirzepatide when there is diabetes or obesity, and newer drugs for particular stages of MASH. Whether any of them suits you depends on how much scarring there is, which is why a fibrosis assessment comes first.
- Do I have to cut out all fat?
- No. The plan uses oil and nuts every day. What matters is the amount, the kind — oil rather than ghee or banaspati — and cutting fried and bakery food.
